How do I review my report?
- Sign in to ID.me and verify your identity.
Select Sign In on the Sign In to View Your Report page. Multi-factor authentication, or MFA, and identity verification are required to access your report. If you have an ID.me account you can use it to access your report. Otherwise, select Create an ID.me account on the ID.me sign in page. You must complete ID.me's process to verify your identity for the NPDB. Once you verify your identity, select the option to allow ID.me to share your information with XMS. This is required to connect your NPDB account to your ID.me account. Learn more about MFA and Identity Verification. - You may be required to complete additional steps for NPDB identity verification.
If you have an ID.me account that is already identity verified with another agency, use that account to sign in to the NPDB. We may be required to collect additional information to comply with our federal security requirements. In that case, please complete all requirements that are displayed when you sign in. - You will need information from the notification letter.
You will need to enter the Account ID from your NPDB notification letter. If your letter has an Access Code, you may also need to enter it. If you do not have the letter, you can request a Self-Query to obtain this information. - After you sign in, verify your contact information.
Review and verify your contact information. You can add another email address to receive NPDB notifications. After verifying your contact information you can view your report.
